What do I do if I'm having chest pains or am short of breath? 
Please proceed to the Bozeman Deaconess Hospital Emergency Room. Activate Emergency Medical Services by 911 if there is concern of heart attack or if symptoms are severe. You will be evaluated by one of the physicians on duty in the Emergency Room. Your personal physician will be contacted and if our assistance is needed they will contact us immediately.

How do I get an appointment with Bozeman Deaconess Cardiology Consultants? 
Since we serve as consultants to other physicians, our practice sees new patients on referral from other physicians. We see established patients by appointment only. Appointments for both our Bozeman and Ennis clinics may be scheduled by calling our office at 406.522.3959 during regular office hours from 8:30 a.m. to 5:00 p.m. or by submitting an appointment request through this website. 

What do I need to bring to my appointment? 
Be sure to bring your insurance card(s), a list of your current medications or the bottles, and any forms we may have mailed to you. Please make sure all forms are filled out completely.

Why do I have to fill out a questionnaire before I am seen in your office? 
The questionnaire is necessary because it supplies the physician with essential information about you. Some of this information will involve your past medical history. Should you have problems with the form, our office personnel will be happy to assist you.
