Diabetes Education
Whether you are recently diagnosed or have lived with diabetes for many years, ongoing education is essential to maintaining good health. The more you know about your diabetes, the more effective you will be in controlling it. The following educational programs are designed using national diabetes education standards to help you successfully manage your diabetes.

Diabetes Self-Management Class
This 2 part class is for individuals interested in learning how to manage their diabetes, care for themselves, and control blood glucose levels. This program has been recognized by the American Diabetes Association (ADA). If you are newly diagnosed with diabetes you should start with this class.

Diabetes Nutrition 101 & Counting Carbohydrates Class
Basic information is covered including when, what and how much to eat when managing diabetes. It also includes basic training on carbohydrate counting including hands-on practice to help understand the impact of food on blood sugar.

Pre-Diabetes Class
This is a two-part class for those diagnosed with Impaired Glucose Tolerance (IGT), Pre-diabetes, Metabolic Syndrome or Insulin Resistance. Learn how and when to monitor blood sugar levels and interpret results, how food affects blood sugar, tips for weight management, healthy food choices and how to begin a safe exercise program.

ActNOW is a comprehensive 10-month weight loss program offering 16 weekly lifestyle classes and 6 monthly follow-up meetings designed to help prevent diabetes and cardiovascular disease, and help you maintain success with a healthy, new standard of living. Our coaches will provide the education and support to help you make lifestyle changes. The program has had proven results averaging a 7% weight loss and 150 minutes activity per week. Eligibility Requirements: Adult Age 18 or overBMI > 25 PLUS one of the following criteria:Abnormal Fasting Plasma Glucose of 100-125Abnormal HgA1c of 5.7-6.4%History of Gestational DiabetesHistory of birth to a baby greater than 9 poundsHigh Blood Pressure (130/85 mmHg or treatment)High Triglycerides (150 mg/dl)High LDL Cholesterol (130 mg/dl or treatment)Low HDL Cholesterol (40 mg/dl men and 50 mg/dl women)Lab work must have been completed within the past 6 months.Participation Requirements: Be motivated to make lifestyle changes through diet and activityAttend the once a week 16 week core sessions Attend the once a month 6-month follow-up sessionsComplete daily food/physical activity logbookCost: $150 ($50 will be refunded to participants who complete the full 16 week and 6 month follow-up program) Benefits: A Life Without Diabetes!
